The term "drug" only has negative connotations when implemented in the context of the abuse of such substances, though depending on the particular drug, the definition of abuse can vary.
Eating one clemintine certainly isn't drug abuse, but eating seventy in an hour is.
Taking one advil isn't drug abuse, but taking twenty is.
Shooting up even a little bit of heroin is drug abuse.
