Well, I don't mean to sound like a doomsayer, but it can get pretty fucking bad. There is this wonderful little infection called gangrene, which is a blood poison I believe. Basically the infected tissue dies and begins to rot. I don't know if there is any modern cure for it, but the traditional cure was amputation or death. As to how long you have, it depends on how clean you keep it and how strong your immune system is.
I also believe that you should either find help, or otherwise get some hobby, something you enjoy, to help keep your mind off self injury.
