This sounds like a pretty good plan to me. It's nice and simple and if she has any feelings for you still it will be warmed welcome. If you do something too over the top she might feel a little pressured to react dishonest for the sake of being nice and not wanting to hurt your feelings/friendship.
If you want to do something a little more, make her a mixtape. Songs that remind you of her or songs you want her to be reminded you of when she listens. Burn a CDR, paint it. Take an old cd case and decorate it with construction paper or whatever else you have on hand. It's unique and she'll probably enjoy it even if she doesn't like a single song you picked out.
