Depends on the sickness. If it's something minor (cold or flu) I'd sleep through it anyway. However, if it's a painful and incurable illness, I'd rather die.

I have nightmares about the spiders so i'll go with the roaches.
Watching someone you love die a slow and torturous death or killing someone you love yourself .

If I had to kill myself, I would choose jumping off a building. There's that whole flying-weightless rush, plus I'd be so high on adrenaline I wouldn't even feel me hit. I guess the main difference between those two is that with the jumping you wouldn't know you were dying, really, and with the drowning you'd feel it painfully until that final euphoria.

If I had some dead flesh that I had to get rid of, I'd pick maggots, but otherwise I'd pick worms. I used to pick up worms all the time, and still do on occasion.
