It shouldn't HAVE to look a certain way, really. Do wahtever the hell you want or think looks good. Screw around with it a bit and you'll find something. I used to wear make up all the time, but it was always done so terribly and messy. I wasn't always going for an elegant look, though. As seen in Figure 1.

If your a fat sweaty metalhead you wont look Fem, people will just think you like Kiss.
If you are small and skinny It might be different.
People somtimes mistake me for a girl for a few ..without eyeliner or anything on.

Personally I hardly ever leave the house without some eyeliner, and whenever I'm in the mood I also apply some eye shadow and/or extend the eyelines to the sides in one way or another.
Nothing too fancy though.
I did try ornaments but found that it looked quite silly on me. The same with lipstick... tried black, grey and dark red, and everything just made me look like a transvestite.

Unless you're Tim Curry, too much makeup can be just plain creepy. It really depends on how feminine the guy looks, but I would recommend that you never wear more makeup than your date. It usually makes them feel uncomfortable.

I stopped using it for a while and now I do when I go to the club. I normally don't when I am just out, mainly for the clubs. I used to do the while white face look but haven't in a while. I'm just lazy, and the goth scene here isn't the same , no one else does , its just lame now.
